ADMISSION GUIDE

  • Please fill out the forms completely and legibly.
  • Present any valid ID with picture and signature.
  • Present authorization letters/ letter of guarantee, ID cards and Medicine / PHIC forms if you are a company patient.
  • Present a note, ID and membership cards from your Insurance Company if you are an HMO patient.
  • Medicare / PHIC members : submit your completed
    Medicare/ PHIC forms to the Medicare / PHIC Section
    ( B.O.2) Ground Floor near billing section)

DISCHARGE GUIDE

  • A written order from the attending Physician is required before the patient can be discharged.
  • Allow at least one hour for the Business Office to finalized your bills.
  • Midicare / PHIC members must submit completed forms to the Medicare / PHIC Section before asking for the Billing Section.
  • Patients with discharge orders must settle their hospital before 12 noon otherwise they will be charged an extra day.
  • The Cashier issues a CLEARANCE SLIP for you to present to the Nurse’s Station for the Charges Nurse Signature.
  • Present signed clearance to guard on duty at exit before leaving the hospital.

REQUEST FOR MEDICAL RECORDS

If you would like to request a copy of your medical records of:
  • Birth Certificate
  • Death Certificate Preparation
  • Issuance of Medical Certificate
  • Release if information for Insurances , HMO’s and other purposes.
  • Retrieval of Patient’s Record for case studies , research ,re –admiited patients.
  • Court Hearing Appearance
  • Statistical Reports Preparation
  • Board Reports Preparation

During Your Stay

ATM Facility
An Automated Teller Machine (ATM) is located in the lobby of the Hospital near the Ambulance parking area.

Dietary Department
Our Dietary Department prepares a variety of top quality, fresh meals. Three (3) complete meals will be provided from which you may select meals according to your taste and dietary requirements.

JULIES FAST FOOD
The fast food offers variety delicious foods , drinks and desserts to cater employees and visitors. Located at basement from 6:00 am – 8:00 pm ( Monday – Sundays)

Coffee Shop
Coffee is located on the 3rd Level of the Hospital . The Coffee shop offers delicious and healthy beverages and a variety of sandwiches and cookies . Open 7 days a week, between 6.00am and 9:0 0pm Monday to Saturday, and between 6am and 7:00pm on Sunday.

Compliments / Complaints
In order for us to serve you the best, we request that you drop the service assess for at the “ Suggestion Box “ near the Information Counter.

Emergency Procedures
The hospital has developed safety and emergency procedures in which each staff member has a major role. In the unlikely event of an emergency, remain by your bed until a staff member arrives to assist you.

Identification on Staff
All staff wear I I.D ‘s as a means of identification and internal security. The I.D’ show the staff member's name and .

Medical Staff
The doctor who admits you is responsible for your medical care whilst you are in Greenslopes Private Hospital. Any questions that you have regarding your treatment should be directed to him or her.

Methods of Payment
We accept the following forms of payment:
  • Cash
  • Credit card (Visa, MasterCard, Diners and American Express)
  • Check payments are accepted but subject to prior validation.

Newspapers
All private rooms are provided a daily local newspaper for free.

Convenience Store and Pharmacy
A convenience store and pharmacy is located in the Hospital's lobby where you can purchase a variety of items including pharmaceuticals, toiletries, newspapers, magazines . Open 24 hours daily, patients and visitors are welcome to utilize this service.

Spiritual Needs
Your spiritual needs wil be provided for by our pastoral care service. Please advise your Nurse for schedule arrangements. A chapel is located at the 3rd Floor operates services daily. Please ask your Nurse for directions.

Television
The hospital provides a television set at your bedside at no additional charge.

Valuables
Cash and other valuable may be deposited with the cashier for safekeeping. Ask for receipt. The hospital is not not responsible for anything lost during your stay.
